Exclusive rights of burial application form

Form CREM 26: some people like to organise their own funeral arrangements to limit the anguish caused to love ones they leave behind. At Great Yarmouth Crematorium you are able to purchase a grave before it is required. In order to arrange this please complete this form

Application for the cremation of the body of a person

Form CREM 1: this form is usually completed by the nearest surviving relative or and executor in order to arrange for the cremation of a person who has died. It should be completed in conjunction with Service Arrangements (CREM 1A) and a Medical Certificate (CREM 4/5).

Service arrangements for crematorium chapel

Form CREM 1A: this form is to be completed to arrange a service to take place at the Crematorium. It should be completed in conjunction with the Application for the cremation of the body of a person (CREM 1) and a Medical Certificate (CREM 4/5).

Medical certificate

Form CREM 4/5: this form needs to be completed by a medical professional. It should be completed in conjunction with the Application for the cremation of the body of a person (CREM 1) and Service arrangements for crematorium chapel (CREM 1A)

Transfer of burial rights

Form CREM 17: a situation often arises where registered owner(s) are deceased and other family members want to arrange for a further burial to take place or for an additional inscription to be placed on the memorial. As stated a living owner(s) is required to give permission for a burial to take place or a memorial to be erected/altered. In order for the burial or memorial request to proceed the Exclusive Right of Burial needs to be transferred to the person(s) entitled to the rights. Use this form to apply for the transfer of rights.

Form CREM 38: use this form to request a genealogy search across Great Yarmouth cemeteries.

Registering a death

Deaths must be registered in person by appointment at one of our registration offices or online via www.norfolk.gov.uk. You should not book an appointment to register until you have been advised that a doctor has issued a medical certificate of cause of death and sent this to the registration service (or the coroner has issued paperwork instead). To book a death registration appointment call us on 0344 800 8020. The registration should be completed within five days from the date of the death, and we ask that you nominate a funeral director before the appointment.

Application to erect and maintain a memorial

Form CREM 22: this form needs to be filled out by those who wish to apply for permissions to erect a new memorial in the cemetery, and for confirming its safety and maintenence.
